Skilled Nursing Visits

Every nursing visit from Hillebrand Home Health is performed by an experienced registered nurse. The registered nurse will work with the patient as instructed by the patient’s physician, providing personalized care that is specific to the physician’s orders. We will develop an individualized plan of care, observe and assess the condition, provide wound care, intravenous infusion, and case management services.

Varied Therapy Options

For patients returning home who have various therapy needs, Hillebrand Home Health will be there every step of the way. Here are some of the therapy options we provide.

Hillebrand Home Health offers physical therapy services provided by a licensed physical therapist or licensed physical therapy assistant. The goal of our therapists is to make the transition from hospital life to home life smoother for the patient. A physical therapy treatment plan with Hillebrand Home Health may include:

  • Therapeutic exercises to improve strength, range of motion, balance, endurance, and reconditioning
  • Individualized exercise programs
  • Pain management assistance after surgery or injury

Hillebrand Home Health speech therapy is provided by a licensed speech therapist. Our speech therapists work with patients to improve swallowing, cognition, and speech rehabilitation after a hospital visit.

For patients that struggle with everyday activities around the home after returning from the hospital, Hillebrand Home Health provides occupational therapy. These activities may include dressing, cooking, and eating. Physical exercises are also used to increase strength and dexterity while other activities help visual acuity.

I.V. home infusion is an alternative to receiving intravenous therapy in a hospital. Patients and doctors often find that at-home I.V. infusion is more convenient and comfortable. Patients can often return to their normal routines while still utilizing I.V. home infusion.
