Clients who are coming home from the hospital may be eligible for nursing or therapy services through their Medicare benefit and/or other insurance carrier. Contact us and we will help you navigate through the process.
Skilled Nursing Visits
A registered nurse conducts our skilled nursing visits. The registered nurse carries out duties by order of the client’s physician. Our registered nurses provide care that is specific to the doctor’s orders. Our professionals will discuss the nature of a disease or illness, observation and assessment of a condition, wound care, intravenous infusion, case management and provide supervision of the client’s care.
Our physical therapy services are provided by a licensed physical therapist or licensed physical therapy assistant. Our therapists work to make the transition from hospital to home easier for the patient.
A treatment plan may include:
- Therapeutic exercises to improve strength, range of motion, balance, endurance and reconditioning
- Creating individualized exercise programs
- Ordering necessary devices or medical equipment
- Massage to decrease pain, muscle spasm, and improve flexibility
- Educating patients about their diagnosis and prognosis
- Home safety and fall prevention review
- Instructing pain management techniques after surgery or injury
Speech therapy is provided by a licensed speech therapist. Our speech therapists educate and assist people in swallowing, cognition and speech rehabilitation.
Our licensed occupational therapists and certified occupational therapist assistants help our clients perform many routine activities. These may include dressing, cooking, and eating.
Physical exercises are used to increase strength and dexterity, while other activities may be chosen to improve visual acuity.
I.V. Home infusion
I. V. Home infusion is an alternative to receiving intravenous (IV) therapy in a hospital. Patients and doctors often find the convenience and comfort of being at home appealing. Patients can often return to their normal routines and even, in some cases, return to work while still on therapy.
